Home and Community Based Waiver
What is the Home and Community Based Waiver?
The Home and Community Based waiver program provides services and supports to the elderly or to adults and children with physical disabilities to help them continue to live in or return to their own home.
It is a state funded Medicaid program. Qualifications are based on income and need. MCHD can assist with the HCBW application process.
MCHD provides HCBW Case Management in the following counties: Clark, Estill, Fayette, Garrard, Jackson, Jessamine, Laurel, Madison, Montgomery, Powell, Rockcastle and Wolfe. Case Management is the coordination of all waiver services to assure the patient can remain safe and healthy in their home with improved quality of life.
Who may qualify for HCBW services:
Persons who are age 65 or older and/or have a physical disability.
Individuals who have a referral from their doctor
Those who are at risk for placement in a nursing home or long-term care facility.
MCHD provides Attendant Care Services in Madison, Estill and Powell Counties. Those attendant care services include:
- Light Housekeeping
- Personal Care Assistance: Grooming, Bathing, Dressing
- Mobility, Transferring, Positioning assistance, Toileting and Incontinence Care, Oral Hygiene
- Shopping/Errand – grocery, post office, etc.
- Medication reminders
- Meal Preparation
- Assistance with feeding
Services provided by Competent Home Health Aides.
